55 year old female patient with a chief complaint of bipedal edema and shortness of breath

55 year old female patient residing in miryalguda farmer by occupation presented to opd with the chief complaint of bipedal edema and shortness of breath since 10 days 

HISTORY OF PRESENT ILLNESS 
She was asymptomatic 1 year back then she got tested positive to corona virus and upon investigations she got reported about kidney problem and she is on conservative management for every 2months 
She has shortness of breath since 10 days which is agravating while walking and bending and relieving while sitting , sleeping 
She has bipedal edema since 10 days which is of non pitting type

PAST HISTORY 
bp since 7 years
Knee pain medication 6 years back
5 years back right hand finger fracture on investigations reported kidney problem

PERSONAL HISTORY
mixed diet before 1year back

FAMILY HISTORY
No significance 

GENERAL EXAMINATION

pallor is seen 
No clubing 
No cyanosis
No icterus 
 Vitals 
Temp 98.4
Bp 150/90
Pulse rate:-  86 b/min
Respiration rate 18 times
Systemic 
Cvs S1 s2 are heard
Cns speech is normal 
Respiratory
Bilateral crepts were heard ifrascapular regions 
Per abdomen 
No palpable mass 
No fluid present
